IV-tPA is the main thrombolytic agent used to treat acute ischemic stroke, but less than 23% of the patients coming to the ER are eligible to receive it and this is mainly related to the short time window for it which is 4.5 hours only.
In the last few years, many trials tried to extend the time window to increase the number of patients receiving and benefiting from it, and the latest European stroke recommendations did extend the time window to 9 hours.

Fatty liver is the accumulation of triglycerides in the liver and within liver cells (hepatocytes). It develops when the percentage of this fat deposition exceeds 5% of liver tissue weight. Non-alcoholic fatty liver disease (NAFLD) is the most common liver disease with a fast-growing prevalence in Saudi Arabia that parallels obesity and diabetes. NAFLD ranges from simple hepatic steatosis to non-alcoholic steatohepatitis (NASH). Most patients experience no symptoms that aggravate the progression of this disease. There is no approved treatment for NAFLD,however, it is important to control the risk factors, including high blood cholesterol, diabetes, and high blood pressure. Also, lifestyle modification and excellent control of co-morbid metabolic illness are urgently needed to prevent the disease progression.
